Our Care Managers provide concierge service, which includes overseeing your care, track your progress, confirm schedules and provide you with community resources.
Caregiver Training

Our caregivers are required to complete an initial and annual training which includes dementia care, safety precautions & fall prevention, emergency procedures, Transfer & Mobility, Assistance with ADLs and more to provide the best experience.
24 Hour Availablity

Our dedicated care team are available 24 hours a day, 365 days a year, including holidays to assist you with your care needs.
Client and Home Assessment

Before starting care with HLHC, our Care Manager will complete an assessment to determine your care needs, challenges, or obstacles that make living at home difficult.
Personalized Care Plan

Our Care Manager will develop a personalized care plan based on your current abilities and ability to perform ADLs. The care plan provides instructions for our caregiver on how care should be delivered.
Caregiver Match

The Care Manager will identify a caregiver who is a match with your care needs. The Care Manager will introduce our caregiver to client before starting care.
